 When you see a doctor or go to a hospital in your plan’s network, you pay a standard office visit copay.

 

 There are no deductibles — your covered services are paid right away.

 

 You have the guidance and support of the best doctors and hospitals in Los Angeles and Orange counties.

 

What Makes Vivity Unique

 

Unlike traditional HMO coverage options, Vivity offers you different ways to access care:

 

 Expanded Choice* offers more access to doctors by letting you request care from certain specialists across Vivity’s network of medical groups. While typical health maintenance organizations (HMOs) only allow members to receive care from their medical group, the Expanded Choice feature gives members the choice of specialty care at partnering medical groups in the Vivity network if certain requirements are met:

  • Approval for the referral is at the discretion of our medical group based on whether the care is medically necessary.
  • The specific specialty requested is offered with Expanded Choice.
  • The specialists at the partnering medical groups can accept Vivity members.

* Members can request a referral from their PCP for specialty care at a partnering Vivity medical group. The specialty care requested must be part of the specialties included in Expanded Choice, must be deemed medically necessary, and the specialists must be contracted to see Vivity patients from another partnering medical group.

 

 

 Urgent care access – You can go to urgent care for common health issues, as well as serious but non-life-threatening conditions such as cold and flu, sprains and strains, or minor cuts that need stitches. Contact your Primary Medical Group and they'll help guide you to the right urgent care center.

 

 Away-from-home guest membership – If you or a family member temporarily live outside of Vivity’s service areas, such as long-term travel for more than 90 days; have separated families; or you need coverage for a dependent enrolled in college, you will still be covered in an emergency. If you need additional care, we can help you find a doctor who is accepted by your plan.

 

*Members can ask their Primary Care Physician for a referral to a Specialist within the Medical Group participating in their Anthem HMO network or to a Specialist outside their Medical Group (i) if such Specialist has agreed to see Members from other Medical Groups and (ii) the member’s Primary Care Physician believes such consultation and/or treatment is Medically Necessary.

Members Have Access to Care, Day or Night

 

Many network doctors offer evening and weekend office hours for convenience. Additionally, Registered Nurses are available by phone for support and answers to health questions through Anthem's 24/7 NurseLine at 1-800-700-0197.

 

Members can access Virtual visits with their PCP or another physician in their Medical Group. After hours video visits are available 24/7 through LiveHealth Online including medical, behavioral health, and other specialty care.

ConditionCare

You or a covered family member can receive help with long-term, chronic health problems such as diabetes, asthma or heart disease. Call 1-855-537-3347 to speak with a ConditionCare coordinator.

MyHealth Advantage

A tool that uses your personal health history to remind you of important preventive screenings, tests, and wellness programs you may not know about.

Future Moms

Building Healthy Families: Formerly Future Moms, this program is available using the Sydney Health app and offers 24/7 personalized, on-demand support to help members grow their families however they choose.
